Hiperglikemia i cukrzyca ciążowa hamują glikolizę łożyska i funkcje mitochondriów oraz zmieniają przetwarzanie lipidów

Stopień, w jakim glikemia matki wpływa na metabolizm łożyskowy komórek trofoblastów [cytotrofoblastów (CTB) i syncytiotrofoblastów (SCT)] u ciężarnych z cukrzycą ciążową (GDM) jest nieznany. Przetestowaliśmy hipotezy, że (a) hiperglikemia hamuje tempo metabolizmu CTB i SCT; i (b) niska aktywność metaboliczna łożyska z łożysk GDM wynika ze zmniejszonego zużycia tlenu przez CTB. Komórki trofoblastów izolowane z łożysk GDM i innych niż GDM terminowe hodowano przez 8 godzin (CTB) i po 72-godzinnej syncytializacji (SCT) w 5 mM glukozy lub 25 mM glukozy. Tempo zużycia tlenu, glikoliza, poziomy ATP i morfometrie kropelek lipidów określono w CTB i SCT.

W CTB z łożysk GDM w porównaniu z CTB kontrolnym: (a) fosforylacja oksydacyjna była zmniejszona o 44% (41,8 w porównaniu z 74,2 pmol O2 / min / 100 ng DNA, P = 0,002); (b) zawartość ATP była o 39% niższa (1,1 x 10-7 w porównaniu z 1,8 x 10-7 nM / ng DNA, P = 0,046); oraz (c) kropelki lipidów były dwukrotnie większe (31,0 w porównaniu z 14,4 µm2 / komórkę, P <0,001) i 1,7 razy liczniejsze (13,5 w porównaniu z 7,9 kropelek lipidów / komórkę, P <0,001).

Hiperglikemia hamowała glikolizę CTB o 55% -60% (średnia różnica 20,4 [GDM, P = 0,008] i 15,4 [nie-GDM, P = 0,029] mpH / min / 100 ng DNA). GDM SCT nie różni się metabolicznie od nie-GDM SCT. Jednak GDM SCT miał znacznie zmniejszoną ekspresję genów związanych z różnicowaniem, w tym hCG, GCM1 i syncytyny-1. Dochodzimy do wniosku, że zahamowaną aktywność metaboliczną przez łożysko GDM można przypisać dysfunkcji metabolicznej CTB, a nie SCT. W trofoblastach GDM dochodzi do obniżenia ekspresji i wydzielania krytycznych hormonów łożyskowych.

Zwiększona lipoksygenazy i zmniejszone metabolity cytochromu P450 skorelowane z występowaniem nefropatii cukrzycowej: Potencjalna rola eikozanoidów z metabolomiki u pacjentów z cukrzycą typu 2

Nefropatia cukrzycowa (DN) jest główną przyczyną przewlekłej choroby nerek i schyłkowej niewydolności nerek. Wcześniejsze badania wykazały, że długołańcuchowe wielonienasycone kwasy tłuszczowe omega-3 (PUFA) mogą mieć terapeutyczny potencjał w zmniejszaniu białkomoczu w DN. Jednak lokalny poziom eikozanoidów pochodzących z PUFA w osoczu pacjentów z DN pozostaje niejasny. Niniejsza praca ma na celu zbadanie różnicy w profilu eikozanoidów w osoczu pacjentów z DN i cukrzycą typu 2 (T2D) bez DN.

W sumie 27 pacjentów z T2D z podobnym czasem trwania cukrzycy zostało zrekrutowanych i podzielonych na grupę T2D + DN i grupę T2D + NDN (nie-DN) na podstawie wykrycia wydalania albuminy z moczem (UAE). Stosując metabolomikę opartą na LC-MS / MS, pacjenci z DN wykazywali podwyższony poziom metabolitów lipoksygenazy (LOX) (5-HETE i LTB4) i obniżone poziomy eikozanoidów pochodzących ze szlaku metabolicznego cytochromu P450s (CYP450) (5,6-DHET) ; 14,15-DHET i 9,10-diHOME). Charakterystyka operacyjna odbiornika i analiza regresji logistycznej ujawniły, że podwyższony poziom metabolitów LOX i obniżony poziom metabolitów CYP450 były istotnie skorelowane z występowaniem DN u pacjentów z T2D. Metabolity LOX i CYP450 korelowały z częstością występowania DN u pacjentów z T2D, co może być celem leczenia DN u pacjentów z T2D.

Skuteczność nowej, wysuszonej błony ludzkiej pochodzącej z owodni, oprócz standardowej opieki w leczeniu owrzodzeń stopy cukrzycowej: randomizowane, kontrolowane badanie pilotażowe z udziałem pacjentów i oceniających

Ostatnie recenzje sugerują, że produkty błony owodniowej mogą przyspieszyć gojenie owrzodzeń stopy cukrzycowej. Nowa wysuszona ludzka błona owodniowa (dHAM) została zastosowana w przypadku owrzodzeń oczu, ale nie w przypadku owrzodzeń stopy cukrzycowej. Było to wieloośrodkowe, prospektywne, zaślepione przez pacjentów i obserwatorów, randomizowane, kontrolowane badanie pilotażowe, w celu zbadania, czy dodanie dHAM co 2 tygodnie do standardowej opieki w porównaniu z samą opieką standardową zwiększyło odsetek wyleczonych owrzodzeń stopy w ciągu 12 tygodni.

Trzydzieści jeden osób (średni wiek 59,8 lat, 81% mężczyzn, 87% cukrzyca typu 2) zostało zrandomizowanych (15 dHAM, 16 osób objętych zwykłą opieką). W ciągu 12 tygodni wygojenie wystąpiło w 4 (27%) owrzodzeniach w grupie dHAM w porównaniu do 1 (6,3%) w grupie zwykłego leczenia (P = 0,1). Procentowe zmniejszenie obszaru rany było wyższe w grupie dHAM w porównaniu z grupą kontrolną. (P = 0,0057). Nie było różnicy w AE między dwiema grupami. Sześciu uczestników przydzielonych do dHAM prawidłowo zidentyfikowało swoją grupę terapeutyczną, chociaż 5 pozostających pod zwykłą opieką błędnie myślało, że znajdują się w grupie interwencyjnej. Ten pilotażowy wynik badania jest zachęcający, pokazując, że ten preparat dHAM jest bezpiecznym i obiecującym leczeniem. Wyniki te zostaną wykorzystane do zaprojektowania statystycznie potwierdzonej, ostatecznej, podwójnie ślepej próby z randomizacją.

Profilaktyka i wczesna pomoc w zaburzeniach odżywiania u młodych ludzi z cukrzycą typu 1

Istnieją dowody na to, że wczesna interwencja jest kluczem do poprawy wyników leczenia zaburzeń odżywiania, podczas gdy podejście „obserwuj i czekaj”, które było powszechne wśród lekarzy rodzinnych i innych pracowników służby zdrowia, jest obecnie zdecydowanie odradzane.

Zaburzenia odżywiania występują około dwukrotnie częściej u osób z cukrzycą typu 1 w porównaniu z populacją ogólną. W tej grupie standardowe leczenie zaburzeń odżywiania przynosi gorsze wyniki, a zaburzenia odżywiania skutkują szczególnie dużym obciążeniem chorobowym. Dlatego naszym głównym priorytetem musi być profilaktyka, z wczesną interwencją tam, gdzie zaburzenia odżywiania już się rozwinęły.
