Czytelnicy badań klinicznych muszą zastosować umiejętności krytycznej oceny, zanim ślepo zaakceptują wyniki i wnioski z badań, aby nie zinterpretować i niewłaściwie zastosować wyniki.
Przedstawiamy podstawy RCT i omawiamy zależność między ryzykiem względnym (RR) a ryzykiem bezwzględnym (AR) pod względem różnych informacji, które każdy przekazuje. Najlepsze wyniki niektórych niedawnych badań sercowo-naczyniowych, w których stosowano inhibitory kotransportera sodowo-glukozowego 2 i agonistów receptora glukagonopodobnego peptydu 1 u pacjentów z cukrzycą typu 2, są wykorzystywane jako przykład zalet oceny zmian RR i AR w celu zrównoważonego przełożenia wyników na sprytna ocena kliniczna.
Sugerujemy również praktyczne wskazówki, które pomogą w klinicznie użytecznej interpretacji raportów zarówno z badań, jak i z różnych badań. Na koniec wspominamy o alternatywnym podejściu, a mianowicie o ograniczonym średnim czasie przeżycia, w celu uzyskania obiektywnych szacunków średniego czasu pominiętych zdarzeń w grupie leczonej w porównaniu z placebo na czas trwania badania.
1ɑ, 25-Dihydroksywitamina D 3 wspomaga osteogenezę poprzez obniżenie poziomu FGF23 u myszy z cukrzycą
1ɑ, 25-dihydroksywitamina D3 (1,25D) i czynnik wzrostu fibroblastów 23 (FGF23) odgrywają ważną rolę w metabolizmie kości poprzez wzajemną regulację. Jednak podstawowy mechanizm między 1,25D i FGF23 w zaburzeniach metabolizmu kości wywołanych cukrzycą nie został jeszcze wyjaśniony. W tym badaniu zbadaliśmy wpływ 1,25D na FGF23 w warunkach cukrzycy zarówno in vitro, jak i in vivo.
Wyniki pokazały, że 1,25D obniża ekspresję FGF23 w osteoblastach znacząco, w sposób zależny od dawki, in vitro w środowisku o wysokim poziomie glukozy. Funkcja komórek beta, insulinooporność i stan witaminy D u pacjentów z cukrzycą typu 2 w zachodniej Kenii
Stan witaminy D w surowicy ma wpływ na stany homeostatyczne glukozy-insuliny leżące u podstaw cukrzycy typu 2 (T2DM). Zostało to opisane w populacji białej i azjatyckiej, gdzie niski poziom witaminy D przewidywał przyszłe upośledzenie funkcji komórek beta i pogorszenie insulinooporności. To badanie miało na celu zbadanie związku między witaminą D w surowicy, insulinoopornością i funkcją komórek beta w subpopulacji czarnych pacjentów z T2DM w Kenii. Głównym celem było określenie poziomu 25-hydroksy (25-OH) witaminy D w surowicy oraz oszacowanie insulinooporności i funkcji komórek beta wśród pacjentów z T2DM w Moi Teaching and Referral Hospital (MTRH). To było badanie przekrojowe.
Do badania włączono 124 pacjentów z T2DM, którzy zgłosili się do poradni diabetologicznej MTRH w okresie od lutego do maja 2016 roku. Wykluczono pacjentów leczonych insuliną i / lub tiazolidynodionami. Uzyskano dane antropometryczne, kliniczne i demograficzne. Do oszacowania wydzielania komórek beta (HOMA-B) i insulinooporności (HOMA-IR) zastosowano model HOMA (homeostatyczny model oceny); podczas gdy wskaźnik dyspozycji {(DI) hiperbola iloczyn wrażliwości na insulinę (1 / HOMA-IR) i wydzielanie komórek beta} został wykorzystany do oszacowania funkcji komórek beta. Zależności między witaminą D w surowicy, insulinoopornością i funkcją komórek beta zbadano za pomocą modelu regresji liniowej. Poziomy witaminy D wykazywały niską dodatnią korelację z DI [r = 0,22 (95% CI: 0,03; 0,37)], ale nie były istotnie skorelowane z HOMA-IR [r = 0,07 (95% CI: – 0,11; 0,25)]. Wyniki te wskazują, że raczej dysfunkcja komórek beta niż insulinooporność jako dominujący defekt wśród pacjentów rasy czarnej T2DM szukających opieki w poradni diabetologicznej MTRH. Wśród nich występuje również niedobór witaminy D, który wykazuje niską dodatnią korelację z dysfunkcją komórek beta. Nie zaobserwowano korelacji między niedoborem witaminy D a insulinoopornością.
Kompleksowe badania psychospołeczne w poradni diabetologicznej dla dzieci
ISPAD zaleca rutynowe, kompleksowe psychospołeczne badania przesiewowe dla nastolatków chorych na cukrzycę. Jednak niewiele klinik wdrożyło procedury zgodne z tymi wytycznymi. W niniejszym opracowaniu opisano wyniki powszechnego, kompleksowego programu badań przesiewowych psychospołecznych w zintegrowanej poradni diabetologicznej dziecięcej zlokalizowanej w akademickim ośrodku medycznym.

Badani: Wśróduczestników było 232 zróżnicowanych etnicznie nastolatków z cukrzycą typu 1 (55,5% kobiet; M wiek = 14,85; 58,5% Latynosów; 20% Czarnych). Młodzież przed wizytą lekarską wykonywała badania przesiewowe na iPadach w poczekalni. Oceniono odsetek nastolatków z dodatnim wynikiem badania przesiewowego w odniesieniu do każdego pomiaru psychospołecznego, a analizy regresji oceniały, w jaki sposób zmienne psychospołeczne odpowiadają za wariancję braku adherencji insuliny i kontroli glikemii (mierzonej za pomocą A1c).
Obawy psychospołeczne były powszechne i wahały się od 7% nastolatków z dodatnim wynikiem badania przesiewowego pod kątem zaburzeń odżywiania i ryzyka samobójstwa do 52% pozytywnych wyników badań przesiewowych z powodu niskiej motywacji do leczenia cukrzycy.
HbA1c i nieprzestrzeganie zaleceń dotyczących insuliny były dodatnio skorelowane z ryzykiem samobójstwa, objawami depresji, lękiem, zaburzeniami odżywiania, stresem cukrzycowym, stresem związanym z monitorowaniem stężenia glukozy we krwi, konfliktem rodzinnym i całkowitą liczbą podwyższeń oraz ujemnie skorelowane z motywacją wewnętrzną.
Nieprzyleganie insuliny, zaburzenia odżywiania, stres związany z cukrzycą i konflikty rodzinne w wyjątkowy sposób przewidywały A1c. Wiek, motywacja i konflikt rodzinny jednoznacznie przewidywały nieprzestrzeganie zaleceń dotyczących insuliny.
